How To Apply

Staying true to the Mission and Vision of The Giving Neighbors we will accept all the applications that meet the requirements. Applicants will be able to apply through the website and via applications received from a volunteer or employee of The Giving Neighbors. Remember, The Giving Neighbors is a nonprofit organization 501 ( c) 3 that is funded and supported by donations. Although an application is received and accepted, funds may not be available to provide assistance.

Please be sure you read these requirements carefully. If all of the information is NOT received the request will not be accepted for review.

To apply for assistance you must fill out the required form at To Apply. You must also provide documents that TGN will use to verify your request for assistance.

In the initial request for assistance you must be very specific about the medical need. Tell us if you need a prescription medications, medical supplies or a doctor's appointment. Tell us whom this request is for. Specify in a detailed paragraph how this need impacts you by being very specific in the choices you have to make. These choices must be between providing for your medical need or choosing between paying for food, shelter or basic living requirements such as utilities.

This is required because The Giving Neighbors works to provide assistance to those who are uninsured and/or ineligible for aid (this tends to be hard working people who have always taken care of themselves and their families). There must be an expectation of an end date for The Giving Neighbors no longer to provide funding or assistance, by being able to go back to work (for example), or working and now eligible to apply for insurance through the company as an employee, or becoming qualified for aid. If you are able to provide some funding towards your needs that is expected.

You must be honest and all information provided must be verifiable. The Giving Neighbors organization will perform due diligence and verify all information received by the applicant.

You will be REQUIRED to provide the following information:

Your full name
Your Home phone and work phone
Your Email address
Your current and verifiable address
Your employer's name
Your employer's address
Your direct supervisor or manager's telephone number and email address
If unemployed, last employer's name, address, contact person, phone number, last date of employment
The amount of your total net monthly income
If self employed; 2 client references, names, phone numbers, email address
The total amount of the expense
The purpose of the expense. Example: doctor's visit, supplies or medications and quantities needed (if applicable)
A full detailed description of the circumstances surrounding the application- Why you need assistance from The Giving Neighbors.

These documents must be received into TGN within 3 days after the initial application is filled out and submitted to TGN. Please make sure that ALL documents are faxed at the same time.

Required:

  1. If a prescription is needed a copy of a current prescription must be presented.
  2. If a doctor's visit is required then a copy of the doctor's invoice, doctor's statement of need, invoice or estimate is required to substantiate the expense or need.
  3. A copy of your driver's license or government issued photo ID.
  4. A copy of any recent Utility bill in your name sent to your legal residence.
  5. A copy of any document showing the amount you pay monthly for your housing and to whom it is paid. A contact name and telephone number of the lean holder or landlord must accompany this information.
  6. A copy of the most recent pay stub for every working member of your household.
  7. If you or any member of your family is receiving Social Security Disability or Social Security Income, a copy to verify these incomes showing your monthly benefit amount must be provided.
  8. If self employed, a balance sheet of your monthly income and expenses; a copy of last two years tax return 

Upon acceptance the request and specific details of your need will be posted at the Neighbors In Need page. This information will show as you have written it.
